Complete the “Phase IV: Monitoring, Maintaining, Supporting, and Evaluating” section of the template. Depending on the system optimization you are proposing, the following items from the maintenance and monitoring section may or may not be relevant. If you are not addressing one of these items, explain why.
1. Address maintenance and monitoring by doing the following:
a. Explain each maintenance task that must be performed and the process for performing each task.
b. Discuss the frequency at which each task must be performed and the schedule of maintenance. Identify the title of the individual(s) who will perform each maintenance task.
c. Describe the method for documenting and storing maintenance records that includes who performed the tasks and when each task was performed.
d. Create a plan for measuring system performance (e.g., effectiveness, efficiency, data integrity).
2. Address supporting and evaluating by doing the following:
a. Discuss how the communication or change theory you selected guided the development of your communication and change management plan.
b. Provide information from your plan on the tools, techniques, trainings, and technologies that will support the user.
c. Discuss the evaluation plan, including how your survey items measure user experience. Complete the “Conclusion” section of the template, including each of the following components:
rationale and purpose of the proposal
summary of how the proposal addresses the gap between the current state of the system and the desired state with system optimization
discussion of why implementing this proposal is important and the impact it will have on nursing practice
summary of how this proposal is significant to nursing informatics and the nursing profession
C. Complete the “Executive Summary” section of the template by summarizing each phase, including key points from your findings as well as details and conclusions from this proposal. Provide no more than three sentences for each phase. Provide a rationale for your proposal.
Note: The key points may be presented as bulleted items.
D. Include each of the following deliverables from your CPE:
maintenance and monitoring plan
communication and change management plan
table of survey items measuring user experience and survey screenshot
E. Create a poster presentation for your optimization project using the phases of an SDLC, and incorporate some of the evidence you created in your CPE by doing the following:
Note: Instructions for creating a poster are provided; you may use any multimedia application (e.g., Microsoft PowerPoint or other equivalent program) to create your poster. Submit your poster as a .pptx or a .pdf file.
1. Support your poster with relevant visuals, including text that gives context to corresponding visuals. The requirements for your poster are as follows:
The poster’s size should be no larger than 24 in. tall and 36 in. wide. (For programs using pixels, this is equal to 2,304 pixels tall and 3,456 pixels wide.)
The minimum font size for titles is 32 and for body text is 20.
Use visuals (i.e., charts, graphs, tables, or illustrations) to clarify and enhance your message. Visuals should cover approximately 50% of the poster area.
Do not overload your poster. Word count for your poster should be 800 words or fewer. Facts should be printed in single phrases or words, not paragraphs.
Center the title of your poster at the top with your name below the title.
Organize your poster into subdivisions (i.e., system optimization purpose, SDLC phases, discussion/conclusion, references).
2. At a minimum, do the following for each subdivision in part E1:
Present your system optimization purpose, discussion/conclusion, and references (with no more than three references).
Present your gap analysis and workflow diagram with a one-sentence description for both the purpose and results for Phase I: Planning and Analysis.
Present your ERD and dashboard visuals with a one-sentence description for each in Phase II: Designing and Building.
Present your SDLC Gantt chart with a one-sentence description. Include a box with no more than five bullets describing tasks related to functional testing for user acceptance testing (UAT) and system integration testing (SIT) in Phase III: Implementing and Testing.
Present a table summarizing the maintenance and monitoring plan with a one-sentence description of the purpose of the table. Include a screenshot of the first page of your survey for evaluating user acceptance, or include a table of 10 survey items for Phase IV: Monitoring, Maintaining, Supporting, and Evaluating.
F. Incorporate the following components of APA style and formatting in your paper:
bias-free language
APA-specific rules regarding verb tense, voice, and perspective
a title page and headers
in-text citations and references
APA-specific formatting rules for margins, spacing, numbering, and indentation for the title page and main body of your paper, including headers, bulleted and numbered lists, and tables and figures
