Quality Improvement Initiative: Enhancing Interdisciplinary Collaboration in Patient Care

In my recent practice setting, a critical quality improvement (QI) initiative aimed to enhance interdisciplinary collaboration to improve patient outcomes. This initiative arose from identified practice gaps, including inconsistent physical monitoring, lack of standardization, inadequate documentation, and minimal interdisciplinary collaboration.

Background and Rationale

Effective interdisciplinary collaboration is crucial in healthcare, ensuring that various healthcare professionals, including nurses, physicians, and allied health professionals, work cohesively to provide comprehensive patient care. The absence of such collaboration can lead to fragmented care, medical errors, and suboptimal patient outcomes (Hall & Weaver, 2001). Thus, the primary goal of this QI initiative was to foster better communication and teamwork among healthcare professionals to address the practice gaps identified.

Nurse’s Role in the Project

Nurses played a pivotal role in this initiative, given their central position in patient care coordination. Key responsibilities included:

Assessment and Planning: Nurses participated in the initial assessment of the current state of interdisciplinary collaboration and identified specific areas needing improvement. This involved conducting surveys and focus groups to gather insights from various healthcare team members.
