The goal of prevention is to lower the hazards and dangers to health. There are three kinds of prevention that consist of primary prevention, secondary prevention, and tertiary prevention. Primary stops the infection before it happens, secondary prevention lessens the effect of a disease after it happens. Finally, tertiary prevention eases the impact of an ongoing illness such as the screening by J.R in the Chinese community.
Identify the different roles and responsibilities of the nurse.
         One of the nurse’s responsibilities is to help patients with chronic diseases change their behavior and adjust their medications according to the guidelines written by practitioners. They lead teams that aim to improve the care given to patients who have high needs and costs and reduce the related expenses (Taylor et al., 2019). They also coordinate the care given to patients with chronic diseases between the nursing home and the rest of the health care system. Moreover, they strive to enhance the health of the population by working with local communities to create healthier environments where people can live, work, learn, and play. The nurse’s role is expanding as evidenced by improved communication with patients, involvement in care management, and autonomy in practice. Nurses also try to address important social factors that may affect the health and well-being of patients with the help of health coaches, care coordinators, and social workers (Taylor et al., 2019).
Challenges, weaknesses and strengths
         One of the most challenging aspects of being a nurse practitioner is dealing with the legal and professional limitations that vary by state and country. There are restrictions on the scope of practice, or encounter negative attitudes from patients who doubt your competence compared to physicians (O’Brien et al., 2021). To overcome this challenge, one needs to be aware of the laws and regulations that govern practice in a particular location. You may also consider relocating to a state or country that allows more autonomy and independence for nurse practitioners. Another challenge is to balance the emotional demands of your work with your personal well-being. As a nurse practitioner, you are involved in the lives of your patients at their most vulnerable moments, and this can be stressful and exhausting (Ryder et al., 2019). One way to cope with this challenge is to do a self-check at the end of each workday and reflect on your feelings and experiences. You can also seek support from your colleagues, friends, or family when you need it. By taking care of yourself, you can also enhance your career opportunities (Ryder et al., 2019). My strength comes from my supervisor and coworkers, who encourage me to use my skills to the fullest extent possible within the law.
Patient population most likely to experience health disparities and why
         Health disparities are closely linked with social or economic factors, such as race, ethnicity, gender, income, education, or geographic location, that create obstacles to maintaining good health. Therefore, the patient population that is most likely to experience health disparities is the one that faces multiple forms of social or economic disadvantage due to racism or discrimination and is underserved in health care. For example, racial and ethnic minority groups, people with lower socioeconomic status, underserved rural communities, and people with disabilities are some of the populations that experience health disparities in the United States. The reason why these populations experience health disparities is because they have less access to quality health care, education, healthy behaviors, and environmental resources that promote good health.
