Instructions
This assignment forms the foundation for a program schedule and structure to elaborate on the program that was illustrated in the logic model. The program evaluation will be designed to address the problem or condition that led to the original need for that intervention. It establishes the framework for all other components of the program planning and evaluation process.
For this assignment, you will create a detailed outline using Word. You will also submit a copy of your schedule that you developed, either in the body of the paper or as an attachment. The tools for the schedule can be from the Gantt Chart Template.xlsx, which is the preferred format. The 31-day format in the Gantt chart can be modified to fit one year.

How long will all the planned actions take?
What actions will be monitored to evaluate if project is succeeding?
Evaluation
What method will be used to determine the success or failure of the program?
How will the proposed project be evaluated? Who oversees this? What, when, and how will it be determined if progress is being/program has been successful?
