Lake Sumter State College Community Health Project – Description

Epidemiologic data from the community/county/state and/or government websites (as appropriate), and current (within the last five years), peer-reviewed journal evidence is collected and reviewed to determine: 1) the characteristics of the identified vulnerable population; 2) why they are considered ‘vulnerable’; 3) the health disparities impacting the population; 4) the social determinants of health perpetuating the disparities; and, 5) the socio-cultural and policy trends and issues negatively impacting the population’s health and well-being.

There are a total of 7 required references for the project and peer reviews from the following categories (students are welcome to use additional sources e.g., websites for epidemiologic data or non-nursing journals that address the topic):

Two (2) Rector & Stanley Chapter citations. The project is supported with a citation from Rector & Stanley (2022) Chapter 23 and a citation from the chapter that discusses the vulnerable population selected;
Five (5) current (within the last 5 years), peer-reviewed NURSING journal articles support the content in the project presentation;

then determine the primary, secondary, and tertiary preventive services needed to meet this population’s health, well-being, and quality of life needs. Once the optimum mix of direct and indirect preventive services are identified, students individually assess their own community for its ability to provide these services.

Collectively, students summarize, analyze, and evaluate the community data collected individually, and determine this vulnerable group’s overall met and unmet needs for direct and indirect preventive care services. As part of the project, your team will identify a public health and/or policy recommendation for change that would benefit the vulnerable population that is the focus of the project. you’ll also identify several stakeholders, decision-makers, or elected officials who are in a position to positively influence the recommendation for needed change.

The community health project has three deliverables:

(1) A narrated PPT presentation of the project (8 – 10 minutes; maximum of 10 slides, excluding the Title and Reference slides). References should be cited in the presentation as appropriate. Rules for effective PPT presentations should be followed (See PPT Resources at the bottom of these directions).
