The client is a female patient name Laquisha Williams. DOB 8/26/1981Dx: HTN, migraine h/a, rheumatoid arthritis, anemia, blood clots, and morbid obesity. She is a friend of my family and a co-worker. The meetings have taken place at lunch breaks at work and after working hours at my office at work. The main reason of helping her would be to improve her mobility by assisting her in managing her weight and to explain to her to manage her hypertension better. She is often non-compliant with her meds.
