Description of the Target Behavior: (should include information regarding how it interferes with learning, severity, frequency, duration, etc.)
Function:
What need does the behavior fulfill for the person? Example: attention, avoidance, control of the situation, venting
Environment:
What is present in the environment that supports the problem behavior?
Replacement:
What behavior should replace the old behavior? Frequency? Duration?
Goals:
Specific goals for intervention defined.
Teaching strategies needed to support the person in developing the new behavior or to diminish the old behavior:
