Getting Started
Adding a form in which to contact someone is a much more efficient and effective way to manage communication through a website. In this assignment, you will be adding a form to your Website Project. Your form should include:
Input box for a name
Input box for an email
A radio button group with at least three choices
Submit button
Cancel button – this should clear the form
Upon successful completion of this assignment, you will be able to:
Create a form and add it to a webpage.
Format a form using CSS.
Instructions
Review the rubric to make sure you understand the criteria for earning your grade.
On one of your website project pages, create a contact form that includes the following elements:
At least one input box for a name
At least one input box for an email
A radio button group with at least three choices
A submit button
A cancel button that clears the form
Validate all of your pages using the validator tools at W3C Markup Validation Service to ensure correct/valid code.
Once you have updated your project page with the form, save the page in your website project folder.
Zip the folder and submit it using the Assignment submission link by the end of the workshop.
